(Is it in-patient, out-patient, or mixed?) ?Provide a rationale for the selected program structure. ?Describe the program location. (That is, is it in a hospital or community, free-standing or co-located?) ?Discuss the staff requirements. (Remember to include types of staff for example nurses, social workers, physicians and so on. It is not necessary to provide a projected budget.) ?Will the program employ individuals in recovery? ?Provide a rational for staffing decisions. ?Detail the program’s evaluation criteria.
