In the modern-day organizations, the two main work environments are administrative and clinical work environments (Wager, Lee & Glaser, 2017). Administrative work environment deals with administrative or financial data and works to provide support for functions of the management and general operations of health care organizations. The clinical environment, on the other hand, contains health-related or clinical information which aids health care providers in the diagnosis, treatment and monitoring of healthcare of patients.
The objectives of the provider of administrative applications for patient administration systems include admitting patients, discharging patients, registering for transfers, scheduling appointments for patients, billing patients for services offered and ensuring proper utilization of resources. In terms of financial management systems, the objectives of the provider are to manage accounts payable, management of general ledger, management of personnel, management of materials, creating payroll and scheduling of staff in the healthcare organization. In terms of clinical applications, provider objectives include ancillary information systems and other clinical information systems (Par, Trudel, Jaana & Kitsiou, 2015). The objectives in ancillary information systems include operations in radiology, laboratory and pharmacy. The provider objectives in other clinical information systems entail documenting nursing activities, maintaining electronic medical record (EMR), providing computerized order entry (CPOE), telemedicine and telehealth, administration of medication and documenting rehabilitation service.
In the examination of the current information system, it becomes evident that the current era is that of transparency, accountability and change which has been made possible through Affordable Care Act. The current information systems in use are mobile applications, cloud computing, social communication through various platforms of social media and next generation analytics.
Workflow Requirements
There are various workflows, communications and protocols requirements that need to be met by a new or upgraded health information system. The first is health information and data which deals encompasses nursing and medical diagnoses, allergies, medical list, clinical narratives, demographics and results of laboratory test. The second requirement is management of results which deals with the management of every result through electronic means. Third, there is order entry and support which deals with the incorporation of the utilization of computerized provider entry (Wager, Lee & Glaser, 2017). Finally, there is decision support which deals entails the use of computerized clinical decision-support capabilities among them being alerts, reminders and computer assisted diagnosis.
The other requirements that need to be met by a new of upgraded health information system are electronic communication and connectivity, patient support, administrative processes, and reporting and population health management. Electronic communication and connectivity help healthcare personnel involved in patient care to achieve efficient communication amongst themselves and with their patients. The right technologies for the facilitation of communication and connectivity encompass web messaging, email and telemedicine. Patient support, on the other hand, entails everything related to patient education materials to home monitoring telehealth. Third, administrative processes help in facilitating and simplifying processes such as verification of insurance, prior authorizations and scheduling (Par, Trudel, Jaana & Kitsiou, 2015). This uses decision-support tolls for the identification of eligible patients to engage in clinical trials or programs aimed at managing chronic diseases. Finally, reporting and population health management serves to establish standardized terminology and the right formats of data in accordance with requirements of reporting of both public and private sectors.
Architecture Alternatives
There are various workflow alternatives, and the first one is the electronic health records (EHR). One of the advantages of EHR are ensuring improved quality, outcomes and safety through computerized alerts and reminder, improved compliance with practice guidelines and reduction of medical errors. The other benefits of EHR are better efficiency, productivity and reduction of costs. Finally, EHR helps in improving quality of services and ensuring better satisfaction. The second workflow alternative is CPOE whose main aim is to improve safety of patients, automation of ordering process and acceptance of orders through electronic means and providing support to actualize proper diagnosis and treatment of diseases (Wager, Lee & Glaser, 2017). However, there are various downsides of the CPOE, and they include complexity of ordering process, the issue of physician entry, the long duration taken to place an order, lack of alignment between incentives and use, lack of confidence in reliability of the system and insufficient training. The third workflow alternative is medication administration which deals with the use of barcodes to enable proper and faster administration of patients, dose, drug, among others. Medication administration has been widely used in various healthcare organizations and has the potential of aiding in ensuring that the right meds are used on the right patients and at the right time.
Provisional Recommendation
With regards to the examination of the workflow alternatives, there are various recommendations to ensure proper working of the system, and the first recommendation is to ensure proper management of vendor access to the leadership of the organization. The second recommendation is to keep process objectives; and third avoid overdoing or under doing request for proposal. The final recommendation is ensuring that leadership team and users are extensively involved in the process of selecting the system.
