A fall prevention program Hip Fracture Prevention has been designed to reduce the morbidity, mortality and costs associated with falls Aim the program To reduce morbidity, mortality and costs associated with falls in 10 residential facilities in Jurassic Aged Care The program has two phases: o Preparation and implementationof project 12 months o Ongoing phase from 13th month onwards Phase one – The preparation and implementation of project 12 months Project Objectives 1. To increase at risk residents balance & strength by 20% within 8 months 2. To reduce falls incidence by 50% within one year 3. To reduce fractured necks of femur by 50% within one year 4. To reduce health care costs for falls by $500,000 within one year Project Strategies 1. To implement a falls risk assessment method to all 10 residential facilities within 3 months; 2.
