Environment: Client lives by himself in a dirty, cluttered trailer. There is no furniture. Kitchen/living area has boxes and a few tools lying around. There is no stove and the refrigerator is not working well. Client uses a microwave and a hot plate or bunsen burner. Support System/Caregiver: Client has a friend that allows Client to come over and wash dishes weekly. His support system is very limited. Medical Status: Client’s health problems include diabetes, COPD, enlarged heart, PVD and HTN. He takes soliqua, metformin, atoravastatin, lisinopril, fish oil and novolog. C has not been hospitalized recently or taken any falls. Functioning Level: C is independent with all ADLs. He stated he uses a rug with a rubber backing in the tub to prevent falls and feels he would benefit from a grab bar. He is also independent with all IADLs except he feels anxious and unsafe driving out of his area of town or getting on the freeway. Mental Status/Coping Skills: Client is alert and oriented X 4. His STM is intact. For the most part Client views life as difficult and people as not trustworthy. He forms a viewpoint and is certain anyone who looks at it differently is wrong. There is no in between. Client begrudgingly agrees to visits but once he gets started he is quite talkative. Socialization: Client states he enjoys getting out and chatting with people but he has no long standing relationships of any depth. Safety Risk: Trailer is equipped with a smoke detector but is in poor repair. He has no water due to numerous leaks in the pipes. He heats with space heaters as does not want to buy propane. The electric only works in part of the trailer. There are no weapons. Client Needs: He is now receptive to assistance with repairs and a deep cleaning. He is willing to 2have his refrigerator replaced. Interventions: CM will make a referral to Home repair requesting assistance with repairing the plumbing and electric systems as well as do a deep cleaning. MORPC will be contacted re a refrigerator and then The Furniture Bank once the deep cleaning is completed. C will continue to receive med transport for those appts located outside C’s comfort zone. C will be case managed at level 2 due to a lack of a support system and the need to track transportation closely because of past abuse of the service. Last Weeks advocacy Please do not duplicate and do similar advocacy. My advocacy chosen is to empower a client by acting on his behalf for the state to step in and take care of him. At present, the son performs the role of a de facto guardian and conservator whose carelessness and neglect causes the health of my client to deteriorate. My client is now placed at a temporary residence offered by the community care center whose care delivery is unsuitable due to his unique needs. In addition, the residence offered by the community is only temporary and is about to expire providing an opportunity for the senior man to be placed in a better nursing home facility that is more responsive to his needs.
