Purpose:The purpose of the community assessment paper is to assess the factors, which influence the health and safety of a community.
Introduction:
Before beginning the Community Survey, read Chapter 15 of the textbook by Maurer and Smith, Community/Public Health Nursing Practice. This will help set the stage for defining and assessing a community. Read the Evaluation Criteria for the community survey assignment. This should be adequate preparation for beginning the community survey
Choose a community, which has meaning and interest to you and have fun learning about it! Be selective and concise; follow the guidelines for the survey.
2. Next, identify persons you can observe/interview and make appointments with them. Your interviews need to be conducted in person so that you can observe and learn about the practice setting during your field experience. You should still make a specific appointment that is convenient for the selected person. (Make sure you send thank you notes after the interview). Prepare a list of questions or possible information you will need from each person. After you have had the interview and gathered other pertinent information (see Community Data Sources), you should be ready to begin writing the paper. Note the ‘possible points’ for each section of the paper. Use the topic areas from the evaluation tool as headings for your formal paper.

Develop an application section for your paper.
The client application section is about a hypothetical client and must be presented in-depth (minimum 2-3 pages). Create a story about the support system and community resources that related to the assessed needs of the client. Provide for all his/her needs using community resources from your survey. Use the P.E.P.S.I.-E. Model to provide a comprehensive assessment of your client’s needs. (See Doc. Sharing for the P.E.P.S.I.-E. model information).
The client can represent one of the scenarios below or any hypothetical patient (do not use personal health identifying information):
a. An 85-year-old widower who has COPD, cataracts and lives alone in a second floor apartment.
b. A sixteen-year-old unwed mother who does not have a car, a baby sitter, or a phone. She lives with her mother who works and is not very supportive.
c. A 28-year-old male with AIDS who lives alone in an apartment.
d. A 40-year-old male quadriplegic who is cared for by his 70-year-old mother. His psychosocial needs are not being met.
5. Format: The paper is to be typed in APA format, limit paper to 12-15 pages. Discuss and reference information (paraphrase) from the interviews in the sections in which it applies. Use APA formatting called personal communication to document the contact with the community representatives. Many large communities now have Web sites with detailed information about the community.
