Needs assessment Develop a community profile of your appointed LGA that includes: i) Basic demographics and information on the following social determinants of health (Early life, Education, Housing, Social gradient, Income, Transport, Unemployment, Work). ii) A summary of your findings from your formative analysis From your group analysis of the profile, identify ONE priority issue that your group plan will address to promote community health and wellbeing. Your needs assessment should also identify any additional information that may be required to complete the profile and sources from which such information could be collected. The plan: The plan to address your priority issue should include: Aim and objectives of the plan Strategies to achieve the plan Resources required including personnel (A realistic budget) A risk assessment of the plan How you will evaluate the intervention A timeline for implementation and evaluation A copy of Criteria for Reports can be found on the Moodle site to assist in the completion of the report. It is also on the eLearning space in the Resources Folder. Individual Component For this component, each student will conduct a health impact appraisal and write a statement outlining the potential impacts of the proposed plan on the health of the LGA population. The appraisal should include both positive and negative impacts and intended and possible unintended consequences.
