The issue of wound drainage has become a subject of discussion and controversy for the last ten years. More austerity techniques and reduced rate of infection have been reported. To prevent the formation of the hematoma, the only preventive measure that is used in the treatment of a wound following a total knee replacement entails the use of a suction drain (Tsang et al., 2016)The treatment is imperative because hematoma may enhance the colonization of bacteria leading to superficial infection. The bacterial may migrate to the wound from the lumen causing an infection known as the deep-seated infection.The rationale for the article-the importance of this subject?This subject is essential in that it equips peri-operative practitioners with adequate knowledge and skills on how to help patients achieve a higher level of wellness like they had before their preoperative journey. Moreover, it strives to highlight both the significance of drains in Total Knee Replacement (TKR) and the adverse outcome that results from drain especially the potential complications that a patient will likely face such as pain and blood loss (Lin et al., 2015).What research I you looked to justify this?Atkinson (2017), acknowledge that drain is an active form of treatment to every patient that receives a TKR. Nevertheless, he concedes that use of drain has high chances influencing complications in the future such as persistent knee pain. Therefore this article is against the use of drain despite the positive outcome it achieves.
Scott (2017) supports the use of drain whereby the author argues that drain plays an essential role of decompressing the wound after the wound closure and deflation of tourniquet. However he argued that the health practitioner administering TKR should be cautious to prevent excessive drainage.
The article by Sharma et al. (2016) focuses on the effectiveness of drains in TKR and is based on research done on 135 knees where some knees received drains while the rest did not receive drains. Drains minimized post-operative ecchymosis but at the same time influenced blood loss. The outcome of this study reveals that there was no added advantage in the use of suction drains over no drain usage because each procedure had its limitations. Therefore this article does not support nor does it dismiss the use of drainage.
The article by Vaidya (2017) analyses past and current studies conducted by different researchers on the topic of TKR and the use of drainage. She found out that drains are frequently used despite their risk of causing bleeding because they reduce the volume of hematoma thus, minimising chances of surgical site infection (SSI), postoperative pain and swelling. Moreover, patients with drainage had a shorter postoperative period compared to those without drains. Therefore this article supports the use of drains because it achieves postoperative wound management.
This article by Zhang (2018) reveals that drains are routinely administered by a large percentage of orthopaedic surgeons despite the risk of blood loss. This article aimed at exposing the benefits of the drain and the result that was obtained revealed there were no superior advantage of drain use. However, this journal supports the use of drains because it explains that patients who did not receive drains have the same chances of blood loss. Therefore it is essential for orthopedic surgeons to continue relying on drains.
The article by Quin et al. (2014) analyzes the use of drains in TKR, and it evaluates the effectiveness of drain over no drains in term of length of hospitality stay, reduction in swelling, and recovery of the knee flexion. The outcome that was obtained from this study revealed that there was no significant difference between drains and no drains. Therefore the authors argue that there is no essence of using drains in that drains did not help in that process of knee recovery after a TKR.
My stand is that orthopaedic surgeons should continue administering drains in TKR because it has high chances of minimizing hematoma formation and shortening the postoperative period among patients with drains. Even though this approach has its limitation such as blood loss, measures to address these shortcomings should be implemented because the use of drain is a crucial method that should be used even in the future.Implications of this researchThis research will provide articulate data about drainage in total knee replacement thus will help to solve the controversy that exists over the use of drains. Also, it will clarify many misconceptions about drainage in TKR thus providing peri-operative practitioners with a peaceful moment where they can easily choose whether to administer drain or not. ConclusionDrainage is essential in TKR especially due to the fact that it minimizes the formation of hematoma. However, this approach has its own limitation because it causes blood loss and if the method is used for a longer period it can have some adverse effect, thus increasing the chances of infection. Therefore one can argue that drain is essential despite its limited benefit something that should compel orthopaedic practitioners to continue administering it.
