Guidelines for writing your case assessment, formulation and treatment plan You are expected to carry out this piece of coursework Case studies have been designed to present realistic problems that exist within varied counselling and clinical populations. Case formulations are a part of the counselling psychology profession. These provide systematic summaries of the most relevant and specific case details as well as information/hypotheses, relating to evidence-based causes and precipitants that are thought to provide a rationale for selecting optimal evidence-based therapy or counselling options. Students could hypothesise causes on the basis of the case material and/or hypothesise research-supported causes that are likely to be of relevance to the case. Case formulations should relate to diagnostic classification and biopsychosocial treatment needs of the patient. You need to offer a critique of diagnoses too. In your assessment and formulation, write about the following (generally in this order): Provide background to the problem by identifying the key factors that you believe to be relevant to the case. This section should be a short paragraph placing the issue in context. Identify relevant, or potentially relevant, individual difference variables, especially those that could connect to the goals and needs of the client. For example, are there specific cognitions and behaviours that could be maladaptive and so could benefit from change? Are there relational factors like attachment and defences, that should be attended to? How could you assess or measure these validly? Reliance on either quantitative and/or qualitative assessments should be considered here. Provide a developmental explanation of the clients presenting problem, exploring, where relevant, problem predisposition, precipitants, and perpetuation. Psychosocial assessment could consider attitudes d beliefs; behaviours (e.g., coping strategies, roles); cognitions, emotional responses, responses to the patient and/or the problem from family or friends (and their attitudes or behaviours); and finally, work issues. Attend to evidence-based theoretical explanations that connect to counselling and therapy options. Identify the predisposing factors. These are the factors in the client’s history that make them susceptible to the problem. Identify the precipitating factors. These are the immediate factors or events that have caused the client to present with or experience symptoms now. Identify the perpetuating factors. These are the factors that are causing the symptoms to continue or progressively worsen. It is also expected that you integrate psychological material from the lectures and independent reading with the formulation of the problem. Particular reference needs to be made to theoretical explanations, especially CBT, humanistic and psychodynamic approaches. Attend to the therapeutic relationship. How is that instrumental in your case formulation and treatment planning. Expand on notions of transference and countertransference. The case formulation should identify any specific treatment implications of what has been described hitherto. Treatment plans could be specified, and choices identified that relate to the varied possible outcomes of the initial choice of therapy. You can use a pluralistic approach if you think it is relevant and justify your treatment choice. (b) How might the philosophy and values of counselling psychology inform research (1000words)? Research in counselling psychology: This is largely a theoretical component. Outline the philosophy and epistemology of counselling psychology and then try to link values and epistemological approaches to the differing research methodologies a counselling psychologist may utilise (i.e reflexivity and qualitative paradigms or the science practitioner model and mixed methods).
